Preparing for Your First Visit
We respect your time and want to make your first visit as pleasant and efficient as possible. Please be sure to bring your:
- Driver’s license
- Insurance card, if you have coverage
- Completed New Patient Forms
Need the Forms?
We can mail our forms to you with a stamped, return envelope or you may download the New Patient forms from this website.
Please complete the forms in ink. Then mail them to us or bring them to your first appointment.
Emergency Care
Dr. Flower treats emergency cases during office hours on the same day they occur. If your emergency occurs after business hours, call the office at 972-306-CARE(2273) for information. We regret that Dr. Flower is not allowed to provide dental advice or prescriptions over the phone for persons he has not seen or treated.
