Patient Forms

To start your first visit with Dr. Flower on time, we need to receive your completed New Patient forms at least 20 minutes before your appointment.  These forms are available as Adobe Acrobat files below.  If you print the forms, please complete them in ink and bring them to your first appointment.

• New Patient Form
• Medical and Dental Health History

Trouble getting the forms to print?  Please call our office at 972-306-CARE(2273).  We will gladly mail the forms to you with a stamped, return envelope.

When you come for your first appointment, be sure to bring your:

• Driver's license
• Insurance cards for you or your family members, if you have coverage
• Completed New Patient forms, if you haven’t already mailed them
